Below you will find various forms and applications that you can view on your computer. You cannot fill the forms out online, you must print them and fill out the paper copy, then mail the forms to us.  Please note that you will need Adobe Acrobat Reader (a free program) to view and print the documents that are in PDF format. You will need to use the Acrobat Reader print icon when printing a form rather than using the internet browser print icon.  The forms will automatically open in your browser window. The file sizes are shown next to the links.

**If you are not a U.S. citizen, do not mail us an application packet directly, you need to work through an international staffing agency (visit the Links page for more information)**

Teacher Application Packet

The easiest and quickest way to receive an application packet is to download one here, print it out, and send it in to the address at the bottom of this page.  Applications should be submitted in a complete package. In order to improve the application process, the application packet, which includes a completed application form with ALL of the requisite supporting documents, must be received in the Division of Human Resources within a 60-day period. Incomplete applications will not be accepted and stray documents will be discarded after sixty days. 

The application packet will guide you as far as all the documents you will need to submit.

To download a Teaching Application Packet:

-First download and print the (226k PDF)

-Then download and print the (240k, PDF)

-Then you have to download and print 3 Professional Reference Forms. There are two types, one is for people who've taught before (including Student Teaching), and the other is for individuals with no previous teaching experience. You can combine the different forms depending on how much teaching experience you have. For example, you can download 1 teaching experience reference forms and 2 of the non-teaching experience forms, based on who your references are.

Next, you can download and print the Background Check Form (43k). Please note that your local Police Department may have a different form to fill out. Contact your local Police Department about their policies.

For teachers who were called for a job interview, you can fill out the preliminary interview assessment sheet and return it to the department by fax. Click here for the form. (57k)
