Below you will find various forms and applications that you can view on your computer. You cannot fill the forms out online, you must print them and fill out the paper copy, then mail the forms to us.  Please note that you will need Adobe Acrobat Reader (a free program) to view and print the documents that are in PDF format. You will need to use the Acrobat Reader print icon when printing a form rather than using the internet browser print icon.  The forms will automatically open in your browser window. The file sizes are shown next to the links.

**If you are not a U.S. citizen, do not mail us an application packet directly, you need to work through an international staffing agency (visit the Links page for more information)**

Teacher Application Packet

The most efficient method of obtaining an application packet is to download one here, print it out, and send it in to the address listed at the bottom of this page.  Applications should be submitted in one COMPLETE PACKAGE. In order conduct a more efficient applications process, the Department of Education Division of Human Resources will not accept incomplete applications.  Application packets that do not contain ALL the required documents will be rejected. The Division of Human Resources cannot guarantee the return of the incomplete application packet.  For your convenience and guidance, the info page (available below) lists all the documents you will need to submit.

If you are applying for more than one position, please provide all the required documents for that application as well, even if those documents were submitted with another application. 

Please include official transcripts obtained from the post-secondary institution, college or university attended. The transcript must be in a sealed envelope addressed to the Department of Education and bearing the name and address of the institution.

Next, you can download and print the Background Check Form (43k). Please note that your local Police Department may have a different form to fill out. Contact your local Police Department about their policies.

For teachers who were called for a job interview, you can fill out the preliminary interview assessment sheet and return it to the department by fax. Click here for the form. (57k)
