Below you will find various forms and applications that you can view on your
computer. You cannot fill the forms out online, you must print them and fill

All Employees Exit Interview Form (17k)-Use this survey if you are retiring or resigning to help the VIDE gather information about its departing employees. Employee Orientation Handbook (3MB) Teachers Praxis Tutorial Login (57k)-Use this sheet to log in to the Plato Praxis tutorial. Teachers who need to take the Praxis I exam have accounts already created for them. If you are a public school teacher that needs to take the Praxis I exam and do not have an account created, please contact the HR office at 773-1095 ext. 358 and 362. Highly Qualified Teacher Identification Form (29k)-All public school teachers need to fill out this form and have a copy at their school and in their personnel files for Federal auditing purposes. Teachers who do not return a completed copy of this form to the Human Resources office may not be eligible for future VIDE assistance such as college courses. Paraprofessionals Parapro Tutorial Login (37k)-Use this sheet to log in to the Plato Parapro tutorial. All paraprofessionals have accounts already created for them. If you are a public school paraprofessional that needs to take the Parapro exam and do not have an account created, please contact the HR office at 773-1095 ext. 358 and 362.
